Report all player/volunteer injuries to the Regional CVPA/Safety Director at [email protected]

For any injury requiring medical attention, the coach MUST complete an Incident Report ASAP and forward it to the Regional CVPA/Safety Director.

Concussion Injury

For concussion injuries, the AYSO Concussion Release Form and AYSO Participation Release Form must be filled out by the physician and signed by the child’s parent/guardian. This is a Michigan State Law and is not an AYSO policy. For a complete matrix of state concussion requirements, click here.

Non-Concussion Injury or Illness

Whenever a player or youth volunteer has suffered an injury or illness that required the care of a physician or a visit to an emergency care facility, the AYSO Participation Release Form must be completed in full and signed by the child's parent/guardian before the player or youth volunteer will be allowed to participate further in the program. 

All completed forms must be returned to the Regional CVPA/Safety Director ([email protected]).
